You must complete our registration forms using the links above and share images of your insurance card PRIOR to your appointment.

IF YOU’VE SEEN US BEFORE :
Please let us know if your insurance, pharmacy or address has changed by texting us ( text name, dob and now information to 205-512-2246)
IF YOU SCHEDULED A TELEMEDICINE APPOINTMENT :
● Our virtual care team must collect any copayment due and new patient forms (if needed) prior to sending you a link to join your telemedicine visit with our providers. Please respond to communication from our team.
● Please make sure you have access to a smartphone/laptop/computer (microphone and camera) with good internet in order to have a successful session. For best results, please use Google Chrome.
● You do not need a username or password. Simply press the given link at the time of your appointment.
IF YOU SCHEDULED AN ANNUAL PHYSICAL :
● Please remember to FAST for 6 hours prior to your appointment time. This means do not have anything to eat or drink except for water for 6 hours prior to your appointment time.
● Note that we can still see you if you forget to fast and we may still be able to do your blood draw regardless, so come in for the appointment anyway.
To cancel your appointment:
All appointments must be cancelled more than 24 hours prior to appointment date and time. The no-show fee for primary care visits is $40.
The no-show fee for endocrinology and dermatology visits is $75.
Telemedicine Psychotherapy appointments must be cancelled 7 days prior to appointment date and time. The no-show fee for psychotherapy visits $75.
If you are late more than 15 minutes to any appointment, you may not be seen and the no-show fees may still apply.
